Before an assessment can be performed a library of questions and templates must be created.
To make changes, right click on an organisation in the Explorer and select Setup Assessment Questions.
By design, changes made to sections, questions and answers will not be reflected in assessments that have already been started. This ensures the meaning of a question cannot be changed mid-assessment. It also ensures questions are not added or removed mid-assessment.
Template actions and control measures are not affected by this; their changes are immediately reflected in both existing and new assessments.
Sections group similar questions together, e.g. Fire, Health & Safety, Legionella. Subsections divide these main sections, e.g. Fire: Sources of Ignition, Fire Detection, Emergency Lighting.
A question appears in its section when performing an assessment and in the assessment’s PDF report.
Deleting a section will delete any subsections and questions and cannot be undone.
Questions can be added to a section or an answer.
In the example at the top of this page, the question “Are heaters used of a more hazardous type?” is a subquestion. This question will only be asked if the question “Are portable heaters used within the premises?” is answered “Yes”.

Deleting a question will delete all answers and subquestions and cannot be undone.
Answers aren’t restricted to Yes and No, you can use any logical response like Satisfactory, Unsatisfactory or Not Applicable. A No answer is not always negative, and a Yes answer is not always positive.

Template actions allow assessors to add common actions to their assessments with a canned description, due date, priority and category. Using templates improves consistency from assessment to assessment and vastly reduces the time it takes to perform assessments, especially when on site using RiskBase Mobile.
One or more template actions can be associated with an answer. In the example at the top of this page, the template action will be available to use if “Are heaters used of a more hazardous type?” is answered “Yes”.

Template control measures are similar to template actions except they are associated with a question — not an answer. Template control measures are available once any of a question’s answers are selected.
