Getting Started

Questions and Templates

Before an assessment can be performed a library of questions and templates must be created.

Example Library Structure

  • Section Fire
    • Subsection Sources of Ignition
      • Question Are portable heaters used within the premises?
        • Answer No
        • Answer Yes
          • Subquestion Are heaters used of a more hazardous type (radiant bar heaters or LPG)?
            • Answer No
            • Answer Yes
              • Template Action Remove all radiant bar and LPG heaters.
      • Question Is the ignition risk from building works properly managed?
        • Template Control Measure Hot work is suitably controlled under a permit to work procedure.

View and Make Changes to the Library

To make changes, right click on an organisation in the Explorer and select Setup Assessment Questions.

By design, changes made to sections, questions and answers will not be reflected in assessments that have already been started. This ensures the meaning of a question cannot be changed mid-assessment. It also ensures questions are not added or removed mid-assessment.

Template actions and control measures are not affected by this; their changes are immediately reflected in both existing and new assessments.

Sections

Sections group similar questions together, e.g. Fire, Health & Safety, Legionella. Subsections divide these main sections, e.g. Fire: Sources of Ignition, Fire Detection, Emergency Lighting.

A question appears in its section when performing an assessment and in the assessment’s PDF report.

Create a Section

  1. Click New Section at the top of the page.
  2. Enter the name of the section in the New Section window and click Create.

Create a Subsection

  1. Right click a section in the data grid and choose New Section.
  2. Enter the name of the section in the New Section window and click Create.

Modify a Section

  1. Double click a section in the data grid — the section’s details will appear on the left of the page. After making changes, click Save.

Delete a Section

  1. Right click a section in the data grid and choose Delete — a confirmation prompt will appear.
  2. Click Confirm and then click Delete.

Deleting a section will delete any subsections and questions and cannot be undone.

Questions

Questions can be added to a section or an answer.

In the example at the top of this page, the question “Are heaters used of a more hazardous type?” is a subquestion. This question will only be asked if the question “Are portable heaters used within the premises?” is answered “Yes”.

Question Text Question wording should be generic where possible; in the example question at the top of this page, “premises” is used instead of “flat” or “office”.

Don’t forget the question mark.

Create a Question or Subquestion

  • Right click a section (or an answer for a subquestion) in the list and choose New Question.
  • Enter the question into the New Question window and click Create.

Modify a Question

  • Double click a question in the list — the question’s details will appear on the left of the page. After making changes, click Save.

Delete a Question

  • Right click a question in the list and choose Delete — a confirmation prompt will appear.
  • Click Confirm and then click Delete.

Deleting a question will delete all answers and subquestions and cannot be undone.

Answers

Answers aren’t restricted to Yes and No, you can use any logical response like Satisfactory, Unsatisfactory or Not Applicable. A No answer is not always negative, and a Yes answer is not always positive.

Negative Checkbox Tick to indicate this answer is negative, e.g. “Yes” is a negative answer to “Are heaters used of a more hazardous type?”.
Negative Description Textarea Shown to the assessor during the assessment but will not appear on the assessment’s PDF report. This field is hidden if “Negative” is unticked.
Template Actions See Template Actions below.

Create an Answer

  • Right click a question in the data grid and choose New Answer.
  • Enter the answer into the New Answer window and click Create.

Modify an Answer

  • Double click an answer in the data grid — the answer’s details will appear on the left of the page. After making changes, click Save.

Delete an Answer

  • Right click an answer in the data grid and choose Delete — a confirmation prompt will appear.
  • Click Confirm and then click Delete.

Deleting an answer will delete any subquestions and cannot be undone.

Template Actions

Template actions allow assessors to add common actions to their assessments with a canned description, due date, priority and category. Using templates improves consistency from assessment to assessment and vastly reduces the time it takes to perform assessments, especially when on site using RiskBase Mobile.

One or more template actions can be associated with an answer. In the example at the top of this page, the template action will be available to use if “Are heaters used of a more hazardous type?” is answered “Yes”.

Category Selectbox Default task category to apply to the action.
Priority Selectbox Default task priority for this action.
Client Settings Window Settings to use when an assessment is performed on a client’s organisation. Task categories and priorities must be selected per client as they are unique to each organisation.

Priority Selectbox See Priority above.
Due Selectbox [Priority Based, Timescale, Ongoing] Default due setting to match the chosen priority’s due setting, a specific timescale (e.g. 1 Month), or ongoing.

Create a Template Action

  1. Double click on an answer in the data grid — the answer’s details including a section titled Template Actions will appear on the left of the page.
  2. Click Action — a New Template Action window will appear.
  3. Once the template’s details are entered, click Save.

Modify a Template Action

  1. Open an answer (see Create a Template Action above).
  2. Double click on a template action in the Template Actions data grid — the template’s details will be displayed in a window.
  3. After making changes, click Save.

Delete a Template Action

  1. Open an answer (see Create a Template Action above).
  2. Right click on a template action and choose Delete — a confirmation prompt will appear.
  3. Click Delete to confirm.

Template Control Measures

Template control measures are similar to template actions except they are associated with a question — not an answer. Template control measures are available once any of a question’s answers are selected.

  2. Click Control Measure — a New Template Control Measure window will appear.
  3. Once the template’s details are entered, click Save.

  1. Open a question (see Create a Template Control Measure above).
  2. Double click on a template control measure in the Template Control Measures data grid — the template’s details will be displayed in a window.
  3. After making changes, click Save.

Delete a Template Control Measure

  1. Open a question (see Create a Template Control Measure above).
  2. Right click on a template control measure and choose Delete — a confirmation prompt will appear.
  3. Click Delete to confirm.
